Registered Nurse

The Hospice registered nurse (case manager) coordinates the plan of care with the physician and hospice medical director through initial and ongoing nursing assessments. The number of visits are determined based on the patient’s needs and can be adjusted as necessary. Medications are reviewed to ensure any distressing symptoms are effectively managed. In addition, the nurse supervises all care provided by the hospice team to confirm patient and family spiritual and psycho-social needs are met.



Education is provided every step of the way. The nurse is responsible for educating the family on how and when to use medications appropriately, along with how to provide physical care for the patient. Education on what to expect as the patient’s disease progresses is provided every step of the way.
