Description

As a Hospice El Paso LVN, you will provide appropriate and adequate vocational nursing care as prescribed by the physician in compliance with applicable LVN licensure and agency policies and procedures to those patients who desire to receive hospice care.

Responsibilities
  1. Has knowledge of Homecare Homebase electronic medical record software.
  2. Trained to work in the Routine Home Care setting as well as the Center for
  3. Compassionate Care general inpatient facility.
  4. Participates in the planning of the nursing care needs of an individual in cooperation with the other IDG members. Participates in the development and modification of the nursing care plan.
  5. Collects data and performs focused nursing assessments of the health status of assigned patients. Provides documentation within 24 hours of assessment.
  6. Assesses and documents current pain level and outcome with every visit. Complete pain assessment minimum of once per week on all patients assigned.
  7. Participates in health teaching and counseling for patient, family, and caregivers as appropriate to overcome identified knowledge deficits, documents in notes and home charts.
  8. Reports changes in patient’s condition to Clinical Manager or Case Manager and physician as appropriate.
  9. Obtains physician orders and implements interventions and changes in treatment as appropriate. Documents phone contacts on the same day.
  10. Monitors medication levels at each visit, ensuring the patient always has a sufficient quantity, and reports need for adjustments to physician to coordinate with patients declining status.
  11. Makes appropriate referrals to other disciplines, recognizing need for additional assistance.
  12. Makes visits at the time of death to support the patient’s family and caregivers following the pronouncement of death by an RN.
  13. Conducts follow-up visits within 24 hours of Start of Care and assists in the evaluation of an individual’s response to nursing interventions and the identification of an individual’s needs.
  14. Covers after hours, weekend call and additional shifts at the Inpatient Unit as needed.
Qualifications
  1. Current licensure as a Licensed Vocational Nurse in the State of Texas.
  2. Current CPR certification.
  3. Two years clinical experience. Background in oncology, care of terminal patients or community nursing preferred.
  4. Working knowledge of nursing procedures.
  5. Personal vehicle available for transportation during work.
  6. Valid driver’s license and insurance as required by the State of Texas.
  7. Must be able to read, write, and speak English at a level which will permit the employee to accurately understand and communicate information as needed to safely and efficiently perform job duties.
  8. Bilingual in the Spanish language preferred.
