Position title

The Hospital Liaison, RN works closely with hospitals, physicians, Admissions staff, and families to assure timely admission of patients referred to Hospice El Paso. Facilitates admissions quickly as a means of increasing referrals and providing continuity of care. Serves as an advocate for patients during the admission process.

  1. Develops a Hospice presence in the community’s hospitals, attending and facilitating rounds and care conferences when possible, conferring with providers, speaking at in-service meetings, attending committee meetings, and participating in new staff orientation.
  2. Develops, educates, and serves as a consultant to hospital medical professionals concerning hospice care, pain management, and hospice Medicare/Medicaid/Commercial reimbursements.
  3. Works in conjunction with hospital case managers in reviewing the hospital daily census (when possible) for appropriate patients, providing consultation and follow up as requested.
  4. Encourages timely consideration by hospital staff and physicians for hospice as an alternative mode of care, assisting in identification of appropriate patients for the purpose of managing pain and symptoms, and providing supportive care for terminally ill patients and families.
  5. Receives and responds to hospice care inquiries and referrals in a timely manner.
  6. Assesses each consult to determine the appropriateness for hospice admission, based on individual hospice criteria.
  7. For patients deemed appropriate for hospice care, conducts a full assessment and according to the established protocol.
  8. Informs appropriate hospital patients and their families about the hospice philosophy, goals, and services.
  9. Works with hospital personnel, patient/family and physician to assure smooth discharge process.
  10. Communicates with Admissions & Marketing staff upon discharge.
  11. Acts as a liaison and advocate for hospice patients who are admitted to the hospital and qualify for hospice care.
  12. Maintains close contact with admissions office and Clinical Managers in between admissions activities.
  13. Performs after-hour admissions as needed.
  14. Follows up on pending and/or non-admit referrals as needed.
  15. Performs triage duties when assigned.
  16. Confers freely with all participating healthcare professionals to facilitate the decision-making process and reinforce collaborative relationships.
  17. Visits various hospitals and facilities to assure that professionals understand the scope of hospice services and admissions criteria.
  1. Valid nursing license in the State of Texas.
  2. Two years clinical experience preferred. Background in oncology, discharge planning, care of terminal patients or community nursing health preferred.
  3. Excellent interpersonal and communication skills.
  4. Personal vehicle available for transportation during work hours and has personal phone.
  5. Valid driver’s license, insurance as required by the state of Texas.
  6. Bilingual in the Spanish language preferred.
