Hospice El Paso is a community based, community supported organization which recognizes that death and bereavement are a natural part of the life process. The Hospice team provides community education and high quality interdisciplinary care designed to meet the physical, emotional and spiritual needs of patients and families who are coping with a life-limiting illness which is no longer responding to aggressive curative therapies.
Who should make a referral?
Anyone may refer a patient to Hospice El Paso. The patient may refer him/herself, or he/she may be referred by a friend, caregiver, relative, social worker, nurse, hospital, nursing home, a Home Health care agency, or the patient's private physician. Our staff is available to visit with the patient and family prior to admission into the Hospice El Paso program for assessment and explanation of the services provided.
After a patient has been referred, the Admissions Staff will contact the patient's private physician to obtain consent to talk to the patient about Hospice El Paso. Information necessary to complete the referral will also be obtained. You may also make patient referrals online by clicking here.
